kindness of the Union Theological Seminary’s Morton Library (Richmond,
Virginia) extending loan privileges to outsiders. Others came from Google Books. Some of those utilized from this source, very
late in the research process, appeared without page numbers and these appear to
be e-book editions and are identified as such.
Why anything bordering on the scholarly would be published—in any lengthy
